En los últimos años, he escrito acerca de las nuevas tecnologías para el tratamiento de enfermedades de la retina, incluyendo el uso de medicamentos (Avastin, Lucentis y Eylea para la DMAE húmeda), el tratamiento con láser (2RT de Ellex – Regeneración de la retina, para la DMAE seca), y el uso de las células madre y terapia génica para una amplia gama de enfermedades oculares.

A principios de este año, tuve conocimiento de una nueva empresa, jCyte, que investiga el uso de células progenitoras de la retina para sustituir fotorreceptores dañados o destruidos y restaurar la visión a quienes cuyos fotorreceptores habían dejado de funcionar, especialmente a quienes se encuentran en las últimas etapas de la retinosis pigmentaria (RP). Yo era consciente de que Advanced Cell Technology también estaba en las primeras etapas para investigar con células progenitoras de la retina. Decidí que la mejor manera de aprender acerca de este singular enfoque para la restauración de la actividad de los fotorreceptores (y tal vez, la visión) para los afectados por RP y otras enfermedades degenerativas de la retina, era llevar a cabo una investigación y escribir sobre ella.

Al hacer la investigación de antecedentes, descubrí que otras dos empresas, ReNeuron y California Stem Cell, también están involucradas en esta área de la tecnología. Esto es lo que he aprendido hasta la fecha.

Introducción

Con el fin de aprender acerca de jCyte, contacté con el Dr. Henry Klassen, que es fundador y profesor asociado del Gavin Herbert Eye Institute y de su Stem Cell Research Center de la Universidad de California, Irvine (UCI), y aprendí acerca de su nueva empresa y sobre cómo va a evolucionar su programa para restaurar la visión a las personas con RP.

Al hacer la investigación básica adicional, descubrí rápidamente que ReNeuron, una compañía de biotecnología del Reino Unido, también está trabajando con el mismo objetivo y, de hecho, estaba trabajando con el Dr. Michael Young del Schepens Eye Research Instititute que resultó haber sido anteriormente co-autor con el Dr. Klassen en los trabajos sobre estudios preclínicos con animales en este campo.

En esta valoración crítica, tengo la intención de explicar que son las células progenitoras de la retina, lo que pueden hacer, y por qué este proceso podría ser una técnica importante para restaurar la visión en las personas con los fotorreceptores dañados o destruidos.

También voy a hablar acerca de las empresas involucradas, en que estado de desarrollo se encuentran y proporcionar un posible calendario para el futuro, incluyendo el trabajo preclínico en marcha y el camino hacia los ensayos clínicos en humanos.

También he incluido algunas informaciones acerca de las actividades comerciales, y en que estado se encuentran estas técnicas/tecnologías alternativas para la restauración de la visión para los que tienen fotorreceptores dañados.

El Problema

Hay un grupo de enfermedades degenerativas de la retina, que constituyen una fuente importante de discapacidad visual, tanto en el mundo desarrollado como en el no desarrollado, y donde las opciones terapéuticas actuales son bastante limitadas.

Por ejemplo, la pérdida de células fotorreceptoras, como se ve en las etapas posteriores de la retinosis pigmentaria (RP), atrofia geográfica (GA) en la DMAE seca, y en las últimas etapas de la enfermedad de Stargardt (SMD), se traduce en la pérdida visual permanente para la que no existe un tratamiento de restauración todavía disponible. Pero, la idea de que las células fotorreceptoras pudieran ser reemplazables en entornos terapéuticos ha recibido un apoyo reciente gracias a trabajos experimentales con modelos animales [1].

La Tecnología

¿Qué son las células progenitoras y qué pueden hacer?

Una célula progenitora es una célula biológica que, al igual que una célula madre, tiene una tendencia a diferenciarse en un tipo específico de célula, pero ya es más específica que una célula madre y, puede ser impulsada a diferenciarse en su célula «diana». La diferencia más importante entre las células madre y las células progenitoras es que las células madre pueden replicarse indefinidamente mientras que las células progenitoras pueden dividirse sólo un número limitado de veces.

La mayoría de las células progenitoras se describen como multipotentes, no pluripotentes. Desde este punto de vista, pueden ser comparadas a las células madre adultas, pero se les llama progenitoras cuando están en una etapa adicional de la diferenciación celular. Están en el «medio», entre las células madre y las células completamente diferenciadas. El tipo de potencia que tienen depende del tipo de su «matriz» de células madre y también de su tipo, en este caso células progenitoras derivadas del ojo y que se han diferenciado parcialmente en células de la retina.

Las células progenitoras de la retina (RPCs)

Las células progenitoras de la retina son células autorrenovables capaces de diferenciarse en los diferentes tipos de células de la retina, incluyendo los fotorreceptores (las células conos y bastones), incluso las células neuronales, y han demostrado ser prometedoras como fuente de células de reemplazo en modelos experimentales de la degeneración de la retina.

Las empresas involucradas

Advanced Cell Technology (ACT)

El programa de investigación principal de ACT utiliza células del epitelio pigmentario (RPE) derivadas de células madre embrionarias en el tratamiento de la DMAE seca, enfermedad de Stargardt y la degeneración macular miópica temprana (MMD). Actualmente la compañía está inyectando estas células hESC derivadas de células madre RPE subretinales en humanos en tres ensayos clínicos, en los que más de 30 pacientes han sido tratados hasta la fecha, sin problemas de seguridad conocidos y, en la mayoría de los casos, con informes de visión mejorada. En un caso, un paciente con DMAE seca, según se informa, mejoro su visión de 20/400 a visión 20/40 después de varias semanas de tratamiento [2]. Creemos que, en este caso, sus fotorreceptores latentes (pero aún con vida) fueron reactivadas por el tratamiento con células RPE.

El ensayo clínico para degeneración miopica (MMD) ha recibido la aprobación Investigational New Drug Application (IND) y se espera que comience en breve.

La empresa también está llevando a cabo los primeros estudios preclínicos con animales en sus laboratorios con varios tipos de células progenitoras.

Según ha informado el doctor Robert Lanza, CSO de la compañía en la última reunión de los accionistas en octubre de 2013 [3]:

«Tenemos, como he mencionado antes, varios tipos de células en el campo del ojo que estamos estudiando, incluyendo nuestros progenitores neurales de la retina, los fotorreceptores progenitores, y también los progenitores ganglionares. Respecto a los progenitores neurales de la retina, hemos visto estas células en animales que tienen degeneración de la retina … y podemos esperar en un rescate muy importante de su actividad».

«También tenemos un programa que estamos llevando a cabo para el uso de estos progenitores de fotorreceptores. Cuando se los inyecta en animales de forma subretiniana, cosa que hemos sido capaces de ver realmente aquí durante la primera semana … las células se incorporan a la retina y en sólo tres semanas pueden verselas moviéndose hacia la capa exterior nucleada e integrándose».

«También estamos estudiando los progenitores de los fotorreceptores. También estamos verificando si podemos recuperar la función visual y la estructura de la retina utilizándolos. También queremos probarlos tanto in vitro como in vivo. También estamos estudiando nuestros progenitores ganglionares y seguimos estudiando los de los animales para prolongar la supervivencia a largo plazo de las células trasplantadas, así como para la protección o la sustitución de las células huésped ganglionares. También estamos intentando usar estas células en el modelo de regeneración del nervio óptico».

Más recientemente, la compañía actualizó el estado de sus programas progenitores en su reciente Form 10-K para el año 2013 [5]:

Programa Progenitor fotorreceptor

Hemos desarrollado una célula progenitora fotorreceptora humana. Creemos que nuestras células progenitoras fotorreceptoras, [derivadas de células madre embrionarias (hESCs)], son únicas en lo que respecta tanto a los marcadores que expresan, así como por su plasticidad: es decir, que pueden diferenciarse tanto en bastones como en conos, y por lo tanto proporcionar una fuente viable de nuevos fotorreceptores para la reparación de la retina. Programa para las células ganglionares progenitoras de la retina

Sólo en los Estados Unidos, aproximadamente unas 100,000 personas tienen el estatus de ceguera legal a causa del glaucoma. El único tratamiento comprobado es la terapia con fármacos o la reducción quirúrgica de la presión intraocular, pero muchos pacientes pierden la visión a pesar de recibir estos tratamientos. En el glaucoma, las células ganglionares de la retina degeneran antes de perderse los fotorreceptores. Actualmente estamos llevando a cabo actividades de investigación y desarrollo pre-clínicos en relación con la diferenciación de las células madre en células ganglionares de la retina y la demostración de la capacidad de estas células para proteger contra la presión intraocular elevada en modelos con glaucoma. Hemos tenido éxito en la generación de una única célula progenitora humana ganglionar que, cuando se inyecta en modelos animales con glaucoma, parece que protege contra los daños y para formar nuevas células nerviosas ganglionares. Vamos a continuar nuestra investigación preclínica en modelos animales, estableciendo una correlación adecuada entre la integración y la función visual en los animales, y luego considerar la preparación de un IND y/o aplicación IMPD para comenzar ensayos clínicos con estas células.

Neuroprotectores Biológicos

En el curso de nuestro trabajo con diversas células progenitoras para el tratamiento de enfermedades degenerativas oculares, hemos descubierto que algunas células progenitoras no sólo tienen la capacidad de participar directamente en la formación de nuevo tejido en el ojo, sino que también eran capaces de ejercer un efecto neuroprotector que reduce la velocidad de la degeneración de los fotorreceptores nativos en los ojos de los animales, por ejemplo, en modelos animales con degeneración macular. Estas células parecían ser también una fuente de factores paracrinos neuroprotectores; agentes biológicos que pueden ser útiles como medicamentos. Además, se observó que estos efectos protectores fueron producidos únicamente por determinados subtipos de células progenitoras. La restricción de esta actividad protectora a un sólo cierto tipo de células progenitoras nos permite examinar los factores producidos diferencialmente por estas células en comparación con otras células progenitoras estrechamente relacionados que no parecen secretar ningun agente de protección. Anticipamos que el/los agente(s) neuroprotector(es) que podamos finalmente desarrollar como candidatos a medicamentos puede(n) ser útil no sólo en enfermedades de la retina y distrofias, sino que tambien pueden tener aplicaciones más amplias en el sistema nervioso central y enfermedades y trastornos del sistema nervioso periférico, incluyendo las enfermedades que causan deterioro de la función cognitiva, trastornos del movimiento tales como la enfermedad de Parkinson, y los episodios isquémicos, tales como las causadas por un accidente cerebrovascular.

Este trabajo de células progenitoras está en las etapas pre-clínicas y aún no está listo para pruebas clínicas humanas.

Californis Stem Cell, Inc. (CSC)

A principios de febrero, California Stem Cell, Inc. (CSC) ha anunciado el inicio de un estudio en colaboración con la Universidad de California, Irvine (UCI), para crear un tejido de la retina 3D transplantable. [6] El estudio, financiado con una subvención de $ 4,5 millones del California Institute of Regenerative Medicine (CIRM), es una continuación de los métodos pioneros de los científicos e investigadores del CSC en la UCI en 2010 [7], e investigará las posibilidades de mejorar la función visual de un paciente mediante el trasplante de células madre humanas derivadas del tridimensional (3D) tejido retinal en su retina.

Hans Keirstead, presidente y CEO de CSC (oficialmente en la UCI) llevara a cabo el trabajo del estudio en la CSC. «Este estudio establece una colaboración valiosa entre nosotros y un equipo de científicos de gran talento de una universidad conocida por su excelencia en la investigación», dijo Keirstead. «California Stem Cell espera hacer una contribución significativa al trabajo que tiene el potencial de ayudar a millones de personas que padecen enfermedades de la retina que alteran la vida».

Este trabajo está en la etapa de pruebas preclínicos muy tempranas con animales.

jCyte

jCyte ha desarrollado métodos que utilizan celulas madre humanas progenitoras de la retina, derivadas de fetos (hRPCs), que se han desarrollado parcialmente en las células de la retina, para activar los fotorreceptores huespedes que han degenerado, y reemplazar y/o reactivar, en este caso, los conos, perdidos por causa de la enfermedad, en las personas con retinosis pigmentaria (RP).

El trabajo de JCyte apuntará inicialmente a las células cono, ya que proporcionan la visión central y la capacidad de leer, conducir y reconocer rostros. (Aunque los bastones también deberían verse afectados.) El trabajo incluirá el cultivo de progenitores de calidad farmacéutica, poner a prueba la seguridad y eficacia en modelos animales, y luego el lanzamiento de un estudio clínico para pacientes con RP muy avanzada, para probar la seguridad y eficacia en humanos.

La investigación de jCyte esta financiada actualmente con varios medios, incluyendo The Discovery Eye Foundation y dos premios del CIRM (the California Institute for Regenerative Medicine), incluyendo $ 4.000.000 del CIRM’s Early Translation II Award [8] y más recientemente una donacion de $ 17 millones de Therapy Development [9]. El trabajo que se está llevado a cabo ahora se centra en la acumulación de las pruebas que necesitamos proporcionar a la FDA para obtener su aprobación. El trabajo previo ha establecido el escenario, demostrando lo que se puede hacer usando estas células, pero ahora hay que repetir todo a una escala mayor, con una extensa documentación, y utilizando las mismas células que se emplearán en los pacientes. Esto se conoce como la fase preclínica del proyecto y, como tal, es la etapa justo antes de comenzar los ensayos. El objetivo principal de la fase pre-clínica es demostrar la seguridad y eficacia del producto en modelos animales, como base para iniciar los estudios en humanos. Si las cosas salen según lo planeado, jCyte debería completar su trabajo de pre-clínica y estar preparado para presentar su NDA a finales de 2014, durante un ensayo clínico en humanos para los pacientes con RP grave. Los pacientes serán inyectados con hRPCs en el ojo mas dañado para determinar la seguridad y eficacia, con suerte, a principios de 2015.

Otras patologías a seguir, una vez que la seguridad y eficacia se demuestren en el estudio sobre RP, podrían ser la atrofia geográfica (GA) que se encuentra en la DMAE seca, y la sustitución de los fotorreceptores que no funcionan en los que sufren la enfermedad de Stargardt (distrofia macular de Stargardt [SMD]).

ReNeuron

Uno de los tratamientos más ambiciosos con células madre que se va acercando a un estudio con humanos está siendo desarrollado por ReNeuron, una empresa del Reino Unido. Su tratamiento con progenitores de la retina reemplaza los fotorreceptores perdidos por la retinosis pigmentaria. Cuando se trasplanten en la retina, los investigadores de ReNeuron creen que las células parcialmente desarrolladas madurarán en fotorreceptores completamente funcionales. La compañía espera lanzar un ensayo clínico este año. Anteriormente, financiado por la Foundation Fighting Blindness, Michael Young, del Schepens Eye Research Institute, Massachusetts Eye y Ear Infirmary, dirigió gran parte de la investigación haciendo este método de tratamiento posible. La designación refuerza el plan de ReNeuron para lanzar una fase I/II de ensayos clínicos para ReN003 a mediados de 2014. Diferentes tecnologías para restaurar la visión a personas con fotorreceptores dañados/destruidos

Además de los programas que utilizan las células progenitoras de la retina, hay varios esfuerzos en marcha para proporcionar visión a aquellos que la han perdido a causa de los fotorreceptores dañados o destruidos.

Voy a enumerar sólo algunas de ellas:

La utilización de células madre – Varias empresas / instituciones se encuentran en ensayos clínicos [16] utilizando células madre para tratar a las personas con la enfermedad de Stargardt, DMAE seca, y retinosis pigmentaria.

El uso de la terapia génica – La terapia génica está también siendo usada activamente [17] en el tratamiento de la DMAE seca (y la exudativa), la enfermedad de Stargardt y la retinosis pigmentaria.

Optogenética – Una rama de la terapia génica, en el que se suministra un colorante fotoactivo a través de un adenovirus en el tejido neural (células ganglionares, sin pasar por los fotorreceptores dañados) y es activado por señales de luz enviadas a estos tejidos activados, que a su vez envían señales eléctricas a lo largo del nervio óptico hasta el cerebro [18]. Varias empresas e instituciones están llevando a cabo actualmente ensayos con animales preparando la realización de ensayos en humanos, entre las que se encuentran Eos Neuroscience, Gensight Biologics, RetroSense Therapeutics, y el Institute de la Vision en París [19], junto con el trabajo que se realiza en Cornell University. por la Dra. Sheila Nirenberg [20].

Los implantes de retina – Varias compañías han desarrollado y actualmente están comercializando dispositivos que se colocan en la superficie de la retina y utilizan cámaras u otros medios ópticos para enviar una señal a los electrodos implantados en la superficie de la retina que a su vez envía una señal al cerebro que simulando inputs visuales [21].

La regeneración de la retina – Un tratamiento con láser mediante el cual se aplica radiación láser en dosis baja en la capa del EPR como un medio para «estimular» las células del EPR para liberar las enzimas que son capaces de «limpiar» la membrana de Bruchs, renovando así la retina y restaurando la visión [22].

Comentario

Personalmente, creo que el uso de células progenitoras de la retina para rejuvenecer o reparar las células fotorreceptoras dañadas en aquellas personas con enfermedades degenerativas de la retina, es un paso importante en la dirección correcta. Si, en los ensayos clínicos programados para comenzar a finales de 2014 o en 2015, esta técnica restaura la visión a las personas que la han perdido debido a los fotorreceptores dañados o destruidos, se convertirá en uno de los grandes avances en la batalla de la lucha contra la ceguera.
